چکیده

Drug-induced cutaneous vasculitis is a known autoimmune complication of tumor necrosis factor (TNF) inhibitors with many instances resulting in the production newly formed antibodies. We report 21-year-old female past medical history Crohn’s disease controlled infliximab who presented to dermatology purpuric rash and crusted plaques her distal lower extremities. Biopsy lesion revealed perivascular lymphocytes, neutrophils eosinophils, vessel wall damage, dermal eosinophils consistent drug-induced vasculitis. Follow up labs assessing for antibodies unusual findings an elevated cytoplasmic-antineutrophil cytoplasmic autoantibody (C-ANCA) titer myeloperoxidase (MPO) antibody level making this patient MPO-C-ANCA positive. The patient’s lesions were treated both oral topical steroids, colchicine, transition ustekinumab subsequent improvement normalization titers.

Myeloperoxidase antineutrophil cytoplasmic autoantibody-associated glomerulonephritis in a very elderly patient with generalized vasculitis at autopsy.

An 86-year-old woman was admitted with hemoptysis and rapid deterioration of renal function. Renal biopsy disclosed necrotizing crescentic glomerulonephritis. Based on positivity for serum myeloperoxidase antineutrophil cytoplasmic autoantibody (MPO-ANCA), MPO-ANCA-associated glomerulonephritis was diagnosed. Steroid pulse therapy was performed, but the patient died after the second course. Aut...

The neutrophil in antineutrophil cytoplasmic autoantibody-associated vasculitis.

Necrotizing, small-vessel vasculitis develops in patients with circulating ANCAs. Neutrophils and monocytes harbor the two major ANCA antigens, PR3 and MPO. A whole body of in vitro experiments implicated ANCA-activated neutrophil effector functions in the pathogenesis of vasculitis, whereas the role of monocytes is less well-characterized. Mouse models for anti-MPO-induced vasculitis were deve...

Epitope analysis of anti-myeloperoxidase antibodies in propylthiouracil-induced antineutrophil cytoplasmic antibody-associated vasculitis

INTRODUCTION Increasing evidence has suggested that linear epitopes of antineutrophil cytoplasmic antibody (ANCA) directed to myeloperoxidase (MPO) might provide clues to the pathogenesis of propylthiouracil (PTU)-induced ANCA-associated vasculitis (AAV). This study mapped epitopes of MPO-ANCA in sera from patients with PTU-induced MPO-ANCA (with or without vasculitis) and primary AAV, aiming t...

HLA-B52-positive Aortitis with Antineutrophil Cytoplasmic Antibody-associated Vasculitis

A 73-year-old man presented with a 1-month history of fever and numbness of the bilateral upper and lower extremities. Laboratory tests showed positive myeloperoxidaseantineutrophil cytoplasmic antibody (ANCA) results (296 U/ mL). Computed tomography (CT) showed wall thickening in the thoracic aorta and common carotid arteries (Picture A), and gallium single-photon emission CT/CT showed accumul...
